UCH Home Health was created to provide an alternative to nursing home or extended hospital stays and to provide continuous, quality care from the hospital into the home. Our professional clinicians work with the patient and physician to provide home health services that meet the specific medical and personal needs of the patient in a compassionate and caring manner.
We are Medicare/Medicaid certified and fully accredited by the Joint Commission. We also accept many insurances and health care plans.
Patients may be admitted to home care from the hospital or other facility as well directly from their physician. In many instances, the careful monitoring and teaching provided by our skilled home care clinicians may prevent initial hospitalization and even re-hospitalization of patients.
Through UCH Home Health Care, located at Tampa at Univeristy Community Hospital, and UCH Home Health of Pasco, a personalized care plan will be developed in collaboration with the physician, the patient and our licensed home care clinicians. The patient will be closely monitored and re-evaluated by our home care team throughout the home care episode. Any changes to the plan of care that are needed will be discussed with the patient and family and approved by the physician.
Members of our home care team include: Registered Nurses, Licensed Practical Nurses, Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers and Home Health Aides. Nutritional guidance is also available from Registered Dieticians. Patients and their caregivers are taught how to effectively and safely provide their home care so that they become an active part of the recovery process.
