Case Management
UCH–Carrollwood's Case Management Department realizes that being hospitalized can create stress and concerns for patients, no matter what their age. The department assists patients and families in making plans for continued care after discharge from the hospital by connecting individuals with appropriate resources to meet their needs. If an individual's medical needs require moving to another care setting, such as a rehabilitation or a long-term care facility, these arrangements will be handled by social workers to make the transition a smooth one. Staff members also assess the social and emotional needs of patients and families to help in their adjustment to an illness or injury. Crisis intervention and counseling are provided to patients and families as the need arises.
